Responsibilities:
Working closely with the Diabetes Educators, Kinesiologist and Endocrinologists, the Social Worker will provide integrated therapeutic practice associated with the needs of individuals with complex diabetes management. You will provide a variety of clinical modalities in accordance with professional standards, clinical policies and evidence-based practices to provide individual, family and group approaches to
Conduct individual, family and group psychosocial assessments and therapeutic interventions associated with barriers to self-management such as fear of hypoglycemia, pediatric transition, and other patient groups
Provided referrals to appropriate community and clinical programs e.g. One Link
Contribute and shape the interdisciplinary care plan
Design, facilitate and evaluate individual and group approaches such as a CBT group
Acts a resource to staff and physicians regarding community and financial resources
Participate in program planning, evaluation and resource development
Qualifications:
Graduate degree in Social Work from a recognized University
Registration and in good standing with the Ontario College of Social Workers and Social Service Workers
A strong knowledge base of diabetes or a Certified Diabetes Educator is an asset
5 years of current counselling/therapy experience in a community chronic disease patient population
Proficient in current assessment, psychotherapy and psychosocial counseling modalities including adaptation strategies to the patient’s learning challenges
Experience in program development, implementation, monitoring and evaluation
Experience and demonstrated ability to work in an collaborative interdisciplinary team environment
Excellent organizational and time management skills
Experienced in the use of computers, various software application including MS Suite and Meditech, electronic charting
Expertise and awareness of existing community and financial resources for referral purposes
Satisfactory attendance record
Able to perform the physical and cognitive demands of the role
Proficiency in a second language considered an asset
Access to a vehicle and a valid driver’s license required
